is to provide quality nursing healthcare and meet residents’ healthcare needs at all costs.

SWHM’ vision for the Intermediate Care Facility

is to provide each resident with a 24-hour continuous active treatment program as soon as the professional interdisciplinary team and facility administrator have formulated a comprehensive individualized resident program plan that specifies intervention and services for each residents’ treatment needs. These residents will include developmentally delayed and cognitive disabilities.
