CONNECT is a network of navigators at clinics, community organizations, and city and county programs. Navigators are community health workers, volunteers, or social workers who link people to services and resources in our community. Agencies in the network are connected through a shared technology platform enabling navigators to send and receive secure electronic referrals, address residents’ social needs, and improve individual and community health.
A public portal on the City and County websites serves as a point of entry to people who self-refer into the network. Navigators can quickly refer people to needed resources. Find more information about CONNECT on the County and City websites.

Santa Fe County Health Services Division promotes the health and well-being of County residents through access to health services and initiatives to support the Santa Fe County Health Action Plan. The Division staffs the Health Policy and Planning Commission (HPPC), an advisory body to the Santa Fe County Board of County Commissioners, providing guidance on health services and programming.The Division oversees CONNECT, addressing the needs of individuals by connecting them with the clinical care and community services they need to stay healthy, leading to improved community health.
