Family Health Partnerships/Companeros de Salud Familiar cares for the individual based on a wholistic model. The Nursing Partnership uses Innovative research and focuses its care upon community based needs. Care is taken in using all community resources by networking with other agencies to ensure all needs of the individual are met. This is a nurse based model, whereby the nurses are focusing on the care of the client. Services provided include prenatal care and follow up of medical assistance with a Community Health Care worker, good for one additional year post pregancy. Services are available for those with Medicaid.
