Family Solutions serves families through an intensive case management model which was adopted from the University of Maryland- Family Connections. Through this model, we complete a 30-day assessment with families to identify strengths and barriers. From this assessment, a service plan is developed collaboratively with the family. The services are 100% client driven. Each family spends at least one hour each week with their case manager focusing on the goals and interventions of their service plan. At 90 days, the case is reassessed and is either closed, due to the successful completion of goals, or extended for up to another 90 days. At the time of successful completion, there is a celebration event with each family to celebrate their successes. All services are free of charge. Funding comes from donations. We do not bill for services or receive any state or federal funding.
Family Solutions provides help to families by providing advocacy and support, connection to resources in the community, and other services. Services are designed for families with at least one child residing in the home between the ages of birth to 14 years. Eligible families should also live within a 45-mile radius of an MCH Outreach office.
The program provides a combination of individual and family services, case management, and advocacy to help the family meet its needs. We help families gain access to community resources that will help their children grow to be healthy and successful at home, at school and in the community.
MCH Family Outreach case managers work with individuals and other people in your family and neighborhood who are important to you. We work with you to identify and build on your family’s strengths to help you meet your needs and those of your children. We meet with you in your home, your child’s school, or anywhere in the community where you are comfortable…as often as needed.
