Ohkay Owingeh Community Health Representatives (CHR)

Objectives:

Provide transportation for non-emergency problems to and from medical facilities

Facilitate communications between community members and medical care providers by providing;

1. Home visits with physical assessments; blood pressure, pulse, etc
2. Assist with making medical appointments
3. Assist with ordering medication, picking-up medication and delivery of medication
4. If community member requests, accompany them into medical provider visit to assist in providing medical recommendations
5. Collaboration efforts to provide community health education, disease prevention (flu vaccines, childhood immuzations) environmental health issues (rabies clinic, animal abuse).
6. Collaboration between community entities: Community School, Head Start,  Elementary School, Diabetes Program, OOBG Club, and the Social Services Program
