Presbyterian Advance Care Planning offers trained advance care planning facilitators to encourage and support a conversation between you, your loved ones and your healthcare provider(s) about what type of healthcare is important to you. Making a plan is important so your loved ones won't have to make crucial healthcare decisions in unexpected or emotional situations.
Our trained facilitators are available to meet with you to support you and answer questions as you work toward creating a personal healthcare advance directive. This is a legal document that identifies what medical support you want or don't want and who can speak on your behalf. This document stays in your medical record and is used in the event you cannot voice your wishes. Only the person you choose to speak on your behalf can make decisions.
