Pathways Program - Centro Sávila

Our Pathways Program works with our clients to connect them to resources in the community, and identify systemic issues that affect their health and well-being such as homelessness, hunger, limited access to needed healthcare, and barriers trying to navigate different systems. Our Pathways Navigator work with individuals with multiple unmet needs and from time to time will do home visits to insure the individuals’ needs are being addressed, or the individual’s might have transportation issues, and/or mobility issues.
