The Medically Fragile Case Management Program provides Registered Nurse Case management/service coordination services statewide for children who are Medically fragile and their families via nine satellite offices. The case managers use a family centered approach to care, and through collaborative partnerships act as advocates, resource persons, facilitators, and paperwork coordinators for families. Case managers support parents as team leaders who address the day-to-day care of their child.
In New Mexico, children who have a high level of dependence on medical technology such as ventilators, tracheostomies, oxygen, feeding tubes, dialysis, intravenous medication and require total care for all their daily needs are identified as medically fragile. The MFCMP program provides community based inhome services utilizing a family centered approach.
