Mi Via Consultant Services

Provide Consultant services to individuals and/or their natural supports who are receiving Mi Via Wavier program services.

What We Offer

  • We explain what Mi Via and/or Centennial Care is to you and what services the program provides;
  • We listen carefully to your needs to create your person centered plan;
  • We help you understand self-determination and empower you to advocate for your needs and/or services;
  • We assist you to locate additional goods and services;
  • We teach you how to access your approved budget;
  • We teach you how to select, hire and train employees and vendors;
  • We explain how to measure your goals and maintain your budget; and
  • We encourage you to advocate for yourself and your needs, and to engage in your community.

Self-Directed Choices staff has experience in resolving Mi Via and/or Centennial Care process issues, including helping to identify to problems with the ultimate goal of enhancing people's lives in their communities.

Self Directed Choices

The mission of Self-Directed Choices, LLC is to open the door of Mi Via Waiver goods and services to all eligible New Mexicans, regardless of age or health condition(s). We accomplish this by understanding each Participant’s disability as it relates to the Participant’s abilities and dreams.

The purpose of Self-Directed Choices is to educate, guide and assist each Participant in clarifying their needs and making informed decisions to their unique Mi Via Waiver. This person-centered process is done in partnership with the Participant’s natural supports, families and other interested parties.
