Senior Care Coordination

Care Coordinators visit you in your home to assess your needs and connect you to services and resources throughout the community. They determine your eligibility for services including home-delivered meals, homemakers, in-home respite care, adult day services, the Senior Companion Program, Silver Alert and medication management. Care Coordinators also provide follow-up and assistance to match services with your changing needs. Care Coordination is available to individuals who live in Bernalillo County and are 60+ and their caregivers.
